Participant Survey

PURPOSE

The purpose of this participant survey is to ensure that CA Care Services is providing services in a manner that supports the individuals needs and goals from the participants perspective. CA Care Services understands that in some cases a participant is unable to complete this survey unassisted and encourages senior support staff to work with them to complete.

If you are a paid carer or support worker, informal carer, or family member of a person with disability, please answer this survey in relation to the person you care for.

When thinking about your experience with CA Care Services, tell us how well you think we are meeting the standards described below. You can do this by telling us to what extent you agree or disagree with the following statements.

Valued - Participants, their families, carers and other support people feel valued in their interaction with the CA Care Services.

Decisions are made on merit - CA Care Services acts in a transparent, informative and collaborative way so that participants understand why decisions are made.

Engaged - 

CA Care Services engages with participants, their family, carers and other support people when developing its procedures and processes.

Supports Needs - CA Care Services utilises outsourced services to ensure that participants have equal opportunity and access to services required in day to day care.

Service Management -

CA Care Services ensures that participants, their family, carers and other support people are involved with their care plan meetings are in making decisions relating to their goals and needs.

Feedback and Complaints - CA Care Services ensures that participants, their family, carers and other support people are given opportunity to send feedback and complaints and they are dealt with in a timely manner.
